IVF Support – Traditional Chinese Medicine, Acupuncture approach for infertility as used by the Chinese for Centuries
            Primarily sterility means that women, who live together with their spouse with normal genital functions for over two years, fail to be pregnant without contraception. Whereas secondary sterility refers to the fact that women fail to be pregnant over two years after previous delivery or miscarriage, without contraception.
MAIN POINTS OF DIAGNOSIS
            Women who are unable to be pregnant for 2 years, or after the previous pregnancy, without normal intercourse and without contraception, can be considered to have infertility provided that her husband has normal genital function.
DIFFERENTIATION AND TREATMENT OF COMMON SYNDROMES
1.      Deficiency of the Kidney
Main symptoms and signs: Infertility long after marriage, delayed menstruation, little and pale menstrual blood, dark and gloomy complexion, lassitude in loins and legs, sexual hypoesthesia, pale tongue with white fur, deep and thread or deep and slow pulse.
Therapeutic Principle: Warming the kidney and nourishing the liver, regulating and tonifying the chong and ren channels.
 
2.      Stagnation of the Liver-qi
Main symptoms and signs: Infertility long after marriage, irregular menstrual cycle, abdominal pain before menstruation, impeded menstrual flow, scanty menstrual blood with dark colour, fullness sensation in the breasts prior to menstruation, irritability and tendency to anger, normal or dark red tongue with white and thin tongue coating, and taut pulse.
Therapeutic Principle: Soothing the liver to regulate the circulation of liver-qi.
 
3.      Phlegm-dampness
Main symptoms and signs: Infertility long after marriage, obesity, delayed menstrual cycle, or even amenorrhea in severe cases, excessive sticky leucorrhea, pallor, dizziness, palpitation, fullness in the chest, nausea, white and greasy tongue coating, and slippery pulse.
Therapeutic Principle: Eliminating dampness and resolving phlegm.
 
MALE STERILITY (Disturbance of sexual function)
            Male sterility (MS) means infertility caused by disorders of male reproductive system. Its clinical manifestations include abnormal sperm such as spermacrasia, aspermia, dead spermatozoon, azoospermia etc, and disturbance of sexual function such as impotence, spermatorrhea, no ejaculation etc. The cause of the disease is rather complex, it may be roughly classified into two kinds: the functional cause and the organic one or the primary one and secondary one. In traditional Chinese Medicine (TCM), MS belongs to the categories of “cold semen”, “scanty semen”, “impotence” and “persistent erection” etc.
            CLINICAL TREATMENT
I.                    Types based on symptom – sign differentiation and their treatment.
Yang Bingxiu classified male sterility into two types:
1.      Deficit due to deficiency of kidney
Manifestations: Lumbago, weak legs, fatigue, lack of strength, clear urine in larger quantities, much urine at night, indifferent sexual desire, long time infertility after marriage, pale tongue with thin-white coating, slender and taut or slender-soft and forceless pulse.
2.      Damp-heat in lower burner
Manifestations: Swelling and cramps in the perineum and lower-abdomen, thirst, bitter taste in mouth, yellowish urine, red tongue with yellow-greasy coating, string-like and slippery pulse.
II.                  Simple recipes and proved recipes
Liu Guilin et al treated infertility of abnormal sperm with Semen-benefiting Decoction.
III.                Other recipes
1.      Acupuncture and moxibustion
Jiang Yuwen applied acupuncture to treat non-ejaculation.
Points selected: Qugu (CV 2), Zuwuli (LR 10), and Sanyinjiao (SP 6).
           
 
Ren Liujiang used acupuncture to treat impotence.
            Points selected: Juyang (at the median of the linking line between Zhinian (BL 54) and Huantiao (GB 30), the needle was inserted obliquely and directed to the opposite symphysis until the patient felt a sensation of numbness, distension and pain at the base of his penis.
            Zhou Qingsheng treated impotence with moxibustion and Qi Gong.
            Manipulation: (1) Moxibustion: Guanyuan (CV-4), Shenshu (BL23) were selected as principal points and Zusanli (ST 36), Sanyinjiao (SP 6) as adjunct points. Moxibustion was given a total of 15-30 minutes, but Guanyuan (CV 4) was ignited with moxa for 5-15 minutes until the local skin of patient became red and hot. Little moxibustion was given to the patient with yin-deficiency and rich fire.
(2) Qi Gong: After moxibustion, doctor pressed his both thumbs on both Shenshu (BL 23) and then applied rolling maneuver of tonification with his small thenar to massage the region of Shenshu (BL 23) for 5 minutes; then the doctor pressed Guanyuan (CV 4) with his index finger, and then evenly and mildly applied rotating maneuver in a clockwise motion to the point with his palm for 5-15 minutes, at a frequency of 90 times per minute. The treatment was given once a day. 12-15 days formed a course of treatment. There was an rest interval of 3-5 days between two consecutive courses.
